In rural Shropshire, the unitary council has developed a joined-up approach between Children’s Services and the housing department. This aims to develop a clear pathway for 16/17 year olds at risk of homelessness.
A screening tool is used to identify early young people at high risk of homeless. All young people presenting to the council have a joint assessment. A social worker is attached to the 16+ Team and does all the Initial Assessments.
Children’s Services also fund a support work to do preventative work with young people and their families. Targeted Youth Support Workers also offer support to young people and families.
